Remove bonus xp (#1089)

* bonus exp removed and awarded through grader

* implemented give bonus exp after tutor is done marking

* moved update_xp_bonus to assessments.ex

* fixed formatting

* removed unnecessary code from grading job

* called decimal_to_integer rather than Decimal.to_integer

* updated documentation

* fixed tests

* bonus xp is awarded even if marker is the same as user

* added new tests

* fixed formatting

* db migration added submitted_at column to submissions

* bonus xp is now calculated from the new submitted_at column

* Redate migration file to maintain total ordering

* Remove extra newline
