re-organization and test reusable blocks, generators, and objects. (#22)

I re-organized some and tried to combine redundant code and refactor some.


* skip some tests

By default the reference design cosimulation tests will not
be included in the test suite (when py.test is run).  The
cosim tests are enabled with the `--include-reference` option.

Also skipped some of the conversion test if ghdl or iverilog
are not present.

* moved things around

Re-organized some of the subpackages and modules.

* testbench reusable components (WIP)

* more temp file clean in makefile, some reorg

* updated tests to use shared functoins and objects

* restoring tb* files

* moved reference designs to test subdir

* moved reference design tests back

* moved reference design tests back

* restored the myhdl install script

* added the __all__ in the __init__s
