Add termination reason to Employees

Why these changes are being introduced:

The 'Termination Type' QB field has been blank until now
but it was determined this can be known for some employees
via the data warehouse data.  HR is currently exploring how
this data can be more widespread for library employees, but
this will at least establish data for some.

How this addresses that need:
* Updates 'employees.sql' to include last appointment transaction
details
* Adds this to Employees table, Termination Type field

Side effects of this change:
* termination types showing up for some employees
