Rework Mox usage in tests

Currently we are providing explicit expectations for 90% of the mocks, and for
the remaining 10% we have two steps of indirection (use FooCase -> stub_with ->
actual mock).
Remove the indirections and just stub in setup/tests where appropriate, so it's
also easier to see what the stubs are returning.
