Add Period and Activity models (#94)

- Adds a `Period` and `Activity` models.
- A `Period` represents a period of time for a given `Campaign`. It can be identified via a `slug` which uses the time period to compute it and then can be used to identify the period externally.
- An `Activity` represents the activity for a given `Period` for a given `address`
