Address Sonar issues

- Replace remaining var uses with let/const and use spread syntax
- Reduce complexity in nested functions, drop unused functions/branches
- Limit upload size
- Combine CSS rule
- Drop "form_is_ready" global browser variable
- Test: Add extra host validation assertion
- Test: MacOS does not seem to provide an error message when connection
  to upstream server is refused, so adjust expected result in test.
- Update README.md and Dockerfile to mention/use minimum tested version
