refactor!: make instruction names more consistent

In particular:
- `xxadd` becomes `xx_add`
- `xxmul` becomes `xx_mul`
- `xbmul` becomes `xb_mul`
- `xinvert` becomes `x_invert`
- `xxdotstep` becomes `xx_dot_step`
- `xbdotstep` becomes `xb_dot_step`

No behavior changes.
