Greatly improved the usefulness of middleware

I made the middleware mutable, which allows middleware to augment the
Virtural Trees in ways that support really cool extensions. For instance
it's very trivial to implement React-like HTML components.

I've also broken down the steps more logically that will make it easier
to document the states:

- Start render transaction (startDate)
- Before syncing (node, oldTree, newTree)
- After patching (patches, promises)
- End render transaction (endDate)
