Commit Message

Add a callback function to indicate when a server becomes failed/recovered.
Server state is based on thresholds applied to the number of consecutive
successes/failures. Connections to failed servers are now automatically
closed down.

Also improve the server selection in the no-rotate case:
- Select a random server if all servers are failed.
- Retry higher priority failed servers with a small probability.
