Ensure jacks don't double up

Assuming that a module has a one to one mapping between jacks and their name (so: there's only one 'out' jack, for instance, or where there are many they're named in some way like 'out1'm 'out2') and that patches aren't using splitters or whatever then we need to make sure that an input or output is only defined once
