fix tag case to stop two tags with different case

It's kind of confusing to have tag 'foo' and tag 'Foo' and tag 'FOO'.
Now we normalize tags to lowercase when comparing for duplicates. In the
big tag list, the first tag's case wins, but each page gets the case
they started with.

Fixes #483
