Fixed Unix2Dos and Dos2Unix filters to protect against double-Flush()

Also make sure that Unix2Dos.Filter() doesn't terminate with an extra '\r'
when ensureNewLine=true and flushing the data when the last character we
saw in the input stream was a '\r'.

This prevents ensureNewLine resulting in \r\r\n.

May be related to issue #1004
