 fix: Append subsection to subsection (#18)

* change logic of section and subsection append now it is work based on filename

* write files even if they are nested

* toc can handle nestd subsection too

* cover add to epub too

* fix golangci errors

* add missed error checking

* fix epubcheck becuse of empty <lo></lo>

* combine functions , return poiners, better comments

* add error handling
