[MODEL] Handle phenotypic inputs with no session column in the TSV (#260)

* add example of no phenotypic session column back to tests

* create phenotypic session if none exist

* remove duplicate participant IDs from example17

* add test for custom phenotypic session when session column is missing

* remove sneaky duplicate row from example4.tsv

* add example of duplicate participant IDs

* add input validation for unique participant-sessions

* test examples with nonunique participant records

* update comments

* remove unnecessary duplicate IDs from example7
