apply: argument required after command

* The usage string suggests the 1st argument after the command name is not optional
* "apply echo 1 2 3" runs echo command 3 times
* "apply echo" by itself is not valid, so invoke usage()
* Exit with failure code in usage() so shell scripts can detect an error for bad options
* I found this when testing against OpenBSD version of apply
