fix(models): Remove static models. (#260)

* fix(models): Remove static models.

  - Also removes extensions (we can now store data inline in the objects themselves)
  - Due to circular dependencies, model files were merged.
  - Lots of cruft cleaned up.
  - Checking for AttachmentModel via inheritance is now pointless, sorted.
  - Update attachment manager model and the corresponding views to use a foreign key for HubuumClass instead of a string...
  - Update all tests and other bits of infrastructure as required.
  - Update documentation to the new reality.
