enh(#30): DS test improvements

- Update ER docs

- Update cmake interface to use RCSW_CONFIG_XX rather than RCSW_XX, no
  disambiguate e.g., RCSW_NOALLOC the configuration item from RCSW_NOALLOC the
  flag.

- Cleanup a number of bugs which popped up for modules implicitly requiring
  RCSW_ZALLOC by explicitly always passing that flag rather than relying on the
  compile-time setting.
