Extensively document IntelliJ + JaCoCo issues

Also added the "Code coverage guidelines" section.

IntelliJ and JaCoCo were driving me crazy. I spent most of the past two
days searching to find out why I couldn't import task.exec files into
IntelliJ and have them show. It ended up being the same problem I saw
when trying to run JaCoCo as part of a run configuration. However, the
error messages were hidden in the IntelliJ debug log.

Extensive details are in the updates to the README from this commit. I
also extensively documented how to diagnose and work around the problem
using IntelliJ's builtin coverage implementation.
