Fix scale-related bugs: stop hangs, and lock contention. (#39)

  - in some cases, magna would hang when stopping flows, and had no mechanism to
    cleanly handle this. this change adds a means to monitor when flows do not
    stop and provide debugging information related to it to the user.
  - the locking in counter generation was fine grained, but complex. this
     approach simplifies locking within flow counters.
