Various fixes found with -Wpedantic.

There are still some outstanding issues with several plugins which
prevent us from building cleanly with -Wpedantic (DENSITY, libdeflate,
and LZHAM), but a bigger problem is C++ didn't add support for
variadic macros until C++ 11, but we need them for the SQUASH_NONNULL
macro.  That means that technically Squash requires C99/C++11…  I'm not
willing to drop SQUASH_NONNULL since it provides very useful
functionality.
