Fixed an issue where control rate values are showing up in timeseries outputs by default. (#987)

* Controls were being added during setup rather than configure which caused control rates to inadvertently end up in the timeseries outputs by default.

* Path and boundary constraints are added to timeseries now if they are ode outputs OR control rates

* Fixed some tests that required control rates to be added to the timeseries outputs.

* fixed more tests that required control rates in the timeseries outputs.

* accommodate older openmdao that didnt provide prom_name with list_outputs by default

* removed a couple of places where static_target was being set and throwing deprecation warnings.
