Add superseded field to course and lesson manifests (#262)

This field contains the lessons and courses that are superseded by
the unit in the manifest. When that unit is mastered, exercises from
the superseded lessons and manifests are no longer shown to the student.

This change does not implement the logic changes, just adds the field.
