Use ActionDispatch MiddlewareStack

This commit revamps the service's middleware stack using
ActionDispatch::MiddlewareStack as a base. This introduces additional
methods for building the stack like insert_before etc.

It also introduces a backward-incompatible change as #use will now
append a new middleware instead of prepending like previously.
