Merge User, Org and Profile models into Account (resolves #1519) (#1697)

This is the most significant change to Funnel's database models since the merger with Lastuser years ago. The User and Organization models have existed separately for over a decade, and the Profile model evolved out of an attempt to create a common namespace for usernames between these two tables, initially starting solely as a username table. We are now merging all three tables into a single model. User accounts and groups of users are now the same table, disambiguated only by a type identification column. Nearly all model-level references to "user" have been replaced with with either "account" when representing ownership, or to more specific role-based names ("created by", "posted by" etc when referencing first party actor roles and "recipient", "member" etc for second/third party target roles).

The term "user" continues to be used in forms and views, where it is pending replacement in future PRs.
