yarpc middleware + error handling (#108)

* Add basic data structure to support middleware

* Add built-in middlewares include ConvertingFilter, ConvertingInterceptor, InboundApplicationInterceptor
And related error handling, and tests.

* Integrate middleware into existing rpc stack flow.

* Update all the examples

* update harness tests including phone, bad-response, unexpected-error endpoint

* Fix the build

* Add python into docker crossdock list

* Add python python-sync into crossdock
