Use new {{UnsubscribeUrl}} link format

This generates the link using the following format, as already
documented in the README:

- https://${EMAIL_DOMAIN_NAME}/${UNSUBSCRIBE_FORM_PATH}?email=<email>&uid=<uid>

With this change, operators are now required to host a page with an
unsubscribe form, updated via JavaScript, described in "Publish your
HTML unsubscribe form."
