vsgi: Throw 'GLib.Error' in 'ApplicationCallback'

Let the server implementation deal with unrecoverable errors.

Replace 'warning' by 'critical' in SCGI.

Remove error handling in 'Router.handle' and forward any error upstream.

Indicate in the documentation that errors are handled by the server
implementation.

Fix subsections for 'Entry point' since we introduce a new subsection
abour error handling.
