Edit, greatly expand README, add bin/gen-test-msg

I wanted to make sure there was at least this much explicit
documentation before announcing it on
https://mike-bland.comhttps://mike-bland.com/. Also tried to reorganize
some existing documentation and headings to make the document more
logical and readable.

Added bin/generate-test-message.sh, as noted in the new README text, to
facilitate sending a first test email immediately after a successful
deployment.
