fix(tlsmitm.go): support TLS handshake with 8.8.8.8 w/o any SNI (#25)

Reference issue: [#2464](https://github.com/ooni/probe/issues/2464)

This diff aims to fix the certificate generation failure caused by
martian in response to an empty SNI by using the impersonated server's
IP and port as fallback SNI.
