Declare a testing extra (as in extras_require) in setup.py

This permits installation of testing dependencies of a local project in
editable mode, e.g. `pip install -e .[testing]`

As a result, we're no longer strictly dependent on pytest-runner, which
has a nasty habit of failing to download and install packages while running
on Travis CI infrastructure (often requiring liberal use of travis_retry)
