Mark tests as skipped if hooks fail

This makes reports more predictable by ensuring Cuppa reports an
outcome for every test the user declares.

This involved refactoring the running code out of Runner into
TestBlockRunner. The new version uses mutable state, in the form
of skipTests, to simplify the logic.
