Allow exclusion of entire files; files by regex.

This introduces two extensions to exclusion files (the -exclude-file
option of the rewriter):

1. Entire source files can now be excluded. This is done by omitting
   the value pattern list:

     file "file/to/exclude"

   Instead of

     file "file/to/exclude" [ value-patterns ]

   Excluded files are not instrumented and do not appear in the
   coverage report.

2. Files to which apply exclusions (whether whole-file or values) can
   be selected by regular expression, by inserting the "regexp"
   keyword:

     file regexp "pattern"
     file regexp "pattern" [ value-patterns ]
