bootstrap sets 'qualify_objects' default with 'db_schema' - fixes #164

The RapidDbic bootstrap (called from rapididapp.pl/rdbic.pl) now
automatically sets the loader-option 'qualify_objects=1' whenever the
user has also supplied a value for the loader-option 'db-schema'. Note
this is just the default, and the user can still set qualify_objects=0
if desired.
