Task/detect multiple instantiations (#186)

* feat: Detect multiple instantiations.

We've seen issues with clients instantiating their SDK once
for every HTTP request and not as a singleton. This PR is a stopgap
solution that counts the number of instantiations per client
identifier.

A client identifier is set to be apiKey, appName, instanceId.
