Improve performance when deleting multiple package versions from non-volatile indices

Removal calls are now batched by index. This avoids most of the volatility modifications to the index, which previously made up about two thirds of all operations on Devpi.

In a benchmark with 120 package versions to remove, index modification took up more than 60% of overall execution time. Batching by index reduces overall execution time in that benchmark from 70 seconds to 25 seconds.
