Add test utilities, fixtures, fixture compiler

Move fixtures out of test cases and into a dedicated fixtures directory.  Move
test-specific utility functions out to tests/include/utils.js. Add test-specific
utility functions for fetching test fixtures, and for compiling them down into a
single CommonJS-compatible module. The compilation step is necessary for running
tests in environments like PhantomJS, where filesystem access isn't available
after entering a page context.
