Remove `Hosts` and `File` bootstrapOptions

They can be replaced by implementing a DiscoverProvider instead. We
provide two default implementations:

1. swim.NewStaticHostList (compatible with old `Hosts` attribute).
1. swim.NewJSONFileHostList (compatible with old `File` attribute).

I did not move the default implementations to the different packages,
because I am trying to limit the scope of this change request.
