Improve 'accept_charset' middleware

Assume the default charset to be 'iso-8859-1' as specified in
'HTTP/1.1'.

Only apply conversion on 'text/*' and use a case-insensitive comparison
for the charset. It's not a convenient to transform non-text data,
especially since it could break binary codings.
