refactor & improve the error system

- New stack_trace dependency to be able to get the current stack trace on demand, and to show a terse & human-readable representation of it.
- Changed throwError() top-level function to a static method ForthError.forth(<int>), that throws errors that will have to be catched upstream.
- New static method ForthError.dart(error) to throw Dart errors.
- Configure QUIT to catch INTERPRET errors and then clear the stacks.
- Replace custom errors and remove several unneeded.
    - Use standard error -13 when the interpreter can't parse a name.
- Update tests for ForthError & Dictionary.
- Update readme.
