Major refactory based on some @trajber ideas

The ideas behind this refactory is to allow the load balancer algorithm to
store the servers list in any format, that could increase the algorithm
performance and make it more flexible for the different scenarios. Other
changes includes:

  * Health check is done only when the servers are retrieved
  * Only healthy servers are injected in the load balancer
  * Server type was removed as it doesn't make sense anymore
