Timeouts should not trigger "multiple remove" log

Timeouts remove message exchanges through a separate path, and so when
the removeExchange is called, no entry is found in the map. This is
expected. Create a separate timeoutExchanges map to track timed out
exchanges so that we don't log the error, and decrement the sendCh
wait group correctly.
