Fix some doxygen comments

Mistakes included:
 - `@param foo[in]` instead of `@param[in] foo`
 - `@param[in:out]` instead of `@param[in,out]`
 - if the `@file` is followed by anything, it is interpreted as a filename. In
   order to refer to the current file, the line following the command must be
   blank.
 - the `@example` command must be followed by a filename containing the
   example. The `@example` commands have been replaces by `@verbatim@ command.
 - some argument names were wrong
 - `@param` commands in group documentation seem to be only used by the first
   documented function in the group.
