Heroku (and other misc changes)

For Heroku, remove fixtures and replace with `seeds.rb`.
Refactor/cleanup test code.
Fix coverage.
Ensure VERSION is always a valid string.
Add Gemfile.lock and setup Gemfile to pin the gem version to 0.dev so it doesn't change.
Update Rubocop so it stops complaining.
Remove Heroku inline buildpack.
Proxy top-level rails/rake commands to the test app.
Setup postgres in production environment (Heroku).
Explicitly define Procfile for Heroku and create release script.
