Use the fetch() API + a polyfill instead of our own XHR wrapper

Use the fetch() API directly or GitHub's polyfill for it
for Chrome < 42, rather than trying to implement our own XHR wrapper.

The proportion of users still using Chrome < 42 is very small
(see https://hypothes-is.slack.com/archives/public/p1452266472000704 )
so we could likely stop including it quite soon.
