[BUGFIX] Throw PackageNotFoundException to avoid fatal error

When extension key is not loaded and PackageFactory attempts to create a FluxPackage instance, a fatal error will be thrown by ExtensionManagementUtility::extPath(). Checking ::isLoaded() before that and throwing an Exception allows Flux to perform the expected fallback to a default and safe package.
