fix frame : w component normalization of LOF transition matrix

this bug was invisible due to the fact that, when p and v are nearly
perpendicular, `norm(p x v) ~ norm(p) x norm(v)`.

The majority of the tests (mainly maneuvers) being on circular orbits
or near an apside of an eccentric orbit, their p and v are nearly
perpendicular.

I should probably include tests along other kind of orbits. For example
I discovered this bug by playing with maneuvers along an hyperbolic
trajectory.
