Issue #19: Fix #19: 100% Unachiveable bug in compute function

This commit fixes the bug of the total percent calculation, adding test
cases to support the issue. The fixture "istanbul-report-100" contains a
report that covers 100% the code from the XML report.

The algorithm now properly computes the 100%.

*	modified:   lib/istanbulReportParser.js
- Removing the "-1" that was decrementing the percent value.

*	new file:   test/fixture/istanbul-report-100.xml
- Adding a test report fixture for a 100% coverage.

*	new file:   test/index-100-test.js
- Adding the test case that uses the fiture and verifies if the computed
  overall percent is 100%.
