Force BASE_ATOMIC_OPS_EMULATED definition if AO_USE_PTHREAD_DEFS

This is for testing purpose only, in case of -D AO_USE_PTHREAD_DEFS is
specified in CFLAGS to Makefile.direct.

* include/private/gcconfig.h [AO_USE_PTHREAD_DEFS
&& !BASE_ATOMIC_OPS_EMULATED] (BASE_ATOMIC_OPS_EMULATED): Define.
* include/private/gcconfig.h [BASE_ATOMIC_OPS_EMULATED
|| USE_PROC_FOR_LIBRARIES && GC_LINUX_THREADS] (MPROTECT_VDB): Merge
two ifdef+define into a single one.
