Fix for issue #58 - synchronization error after a database error

The hole caused the settle() method to mistakenly think that all threads were blocked on the queue.
Tightened the locking around the queue to close the hole.
Ran tsunami for 4 hours without issues. Will continue to run to look for more issues.
