Turn off app preload in Puma

Why:
When Puma starts on Heroku, the following warning appears:
 ```! WARNING: Detected 4 Thread(s) started in app boot:
```
According to Evan Phoenix, this is because the app is using both
preload_app! and cluster mode. To remove the warning, he says you
either have to turn off preloading or recreate the threads in the
`on_worker_boot` block. Since these warnings point to third-party gems,
I'm not sure how to recreate those threads, so I'm turning off
preloading. I tested it on Heroku and it seems to work fine.

Also, from the puma README:
```
General rule is to use preload_app when your workers die often and need
fast starts. If you don’t have many workers, you probably should not
use preload_app.
```
